Asthma Care Program
  • Assessment of patient condition and ongoing communication with a Nurse Care Coordinator
  • Education on spacer usage with inhalers
  • Environment assessment and modification plans
  • Peak Flow meter usage
  • Education on identification of triggers and how to manage them
  • Homecare coordination for hands on teaching and training
Outcome: Increased compliance with physician’s plan of care. Reduced hospital admissions and Emergency Room visits; improved quality of life and a higher understanding of how to best manage their asthma.

Diabetes Care Program
  • Assessment of patient condition
  • Participants attend free Diabetic Education Classes
  • Educate patient on dietary needs and how excess weight can lead to complications
  • Participants receive a free annual dilated eye exam to monitor for complications associated with Diabetes
  • Ongoing communication with a Nurse Care Coordinator
  • Require routine lab tests to monitor condition and adhere to the standard of care for Diabetes
Outcome: Reduced disability time for employees, higher quality of life and ability to participate in an increased number of normal physical activities. Ensure that the patient is following treatment plans appropriately and that they perform the necessary monitoring of their Diabetes.

Coronary Artery Disease Care Program
  • Assessment of patient condition with ongoing communication with a Nurse Care Coordinator
  • Encourage physical activity, when appropriate to improve symptoms associated with heart disease
  • Educate the patient on the long term effects that healthy behaviors can have on their condition
  • Education on how to watch for signs and symptoms of a worsening condition
  • Encourage medication regimen compliance
  • Education on the importance of regular physician visits and lab tests
  • Discuss and encourage ongoing healthy dietary choices
Outcome: Reduced hospitalizations, stabilization of the patient’s condition, decreased progression of the disease, reduced emergency room visits, improved patient participation in the management of their disease, improved patient well-being.

Chronic Obstructive Pulmonary Disease Care Program
  • Assessment of patient condition and current treatment plan
  • Enhanced benefits to ensure patient is compliant with physician’s treatment plan, including prescription regimen
  • Encouraging and supporting the patient’s smoking cessation efforts
  • Educate patient on risk factor identification and management to minimize exacerbations
  • Ongoing communication with a Nurse Care Coordinator
  • Educate patient on correct techniques to optimize respiratory status
Outcome: Reduced disability time for employees, higher quality of life and ability to participate in an increased number of normal physical activities. Ensure that the patient is making every effort to quit smoking and that they perform the necessary monitoring of their COPD.

Congestive Heart Failure Care Program
  • Assessment of patient condition
  • Educate patient on importance of daily monitoring of weight fluctuations
  • Ongoing communication with a Nurse Care Coordinator
  • Education on how to watch for increased signs and symptoms of a worsening condition
  • Encourage medication regimen compliance through enhanced benefits
  • Education on the importance of regularly scheduled physician visits
  • Discuss ongoing healthy dietary choices
Outcome: Reduced hospitalizations, stabilization of the patient’s condition, decreased progression of the disease, reduced emergency room visits, improved patient participation in the management of their disease, improved patient well-being.
